My personal advice as a plumber - call a pro on this one. If your nearest vent is the vertical 3” stack above the sanitary T your toilet is dumping in to… you have my sympathy If you are REALLY lucky that 2” line closest to the camera running perpendicular to the joists is a vent line. You need to identify what plumbing is “vent”. I bet if you used a shop vac and sucked all the water out of the sink’s p-trap and cleared any blockage of the drain line that the toilet would work correctly. Does any water come out of the glass? No, because it’s not vented

Turn the glass full of water open end down in the tub and lift it until the bottom of the glass above the surface of the water. Take a glass, put it in a tub full of water. You flush the toilet, water fills the pipe and negative pressure is created preventing to movement of waste through the pipe. edit * I see that 2” pipe is a sink drain.I don’t think adding a 1” drop is going to solve your problem. That’s less than a 1” drop over your 42” run. Where does the 2” line go coming off the elbow from the toilet? That should be going to a vent stack. This sounds like more of an issue of the drain line not being properly vented. Is fixing this sloping problem a project I can tackle as a mid-level DIYer? The ceiling and inner walls are not an issue and will be reconstructed when this issue is dealt with. Thankfully there is only the basement foundation wall to consider. Are there strategic cuts and replacements I can make or are there things to avoid and watch out for given the configuration in the pictures? I have already measured, cut, and assembled ABS piping a few other times when it wasn't crazy complicated. For years this toilet needs 2 or 3 flushes to swirl and flow freely rather than fill the bowl and drain out slowly. I'm looking for practical advice to fix a toilet flushing problem. I scraped it open again to allow a 1/2' flow and multiple tests have confirmed the awesome power of this old mega tank toilet. It was the flush jet (siphon jet?) at the bottom of the bowl that had become almost completely encrusted with hard water buildup.
